Job Overview

The Community Support Specialist plays a crucial role in providing guidance and support to clients, addressing their personal, social, health, and economic challenges.

Position Details

Location: Newark, Delaware
Schedule: Monday to Friday, 8 AM - 4:30 PM
Compensation: $20 per hour

Key Responsibilities
  • Engage with individuals facing disabilities and the elderly population.
  • Conduct assessments, develop plans, implement services, and evaluate options to fulfill clients' human service requirements.
  • Formulate recommendations as part of a comprehensive Client Service Plan.
  • Interpret relevant laws, regulations, standards, and policies.
  • Prepare detailed narrative reports.
  • Assist clients in document collection and oversee the process until a decision is reached.
  • Provide referrals and support clients in accessing various services and programs.
  • Manage a caseload and assess ongoing eligibility for services under the Community Services Program.
  • Ensure timely completion of assigned activities.
  • Utilize the Aging & Disability database for case management tasks.
  • Respond to routine inquiries within two business days.
  • Monitor new referrals and contact participants to facilitate application processes.
Qualifications
  • A minimum of 6 months of experience in Case Management.
  • Strong narrative writing capabilities.
  • Proficiency in Microsoft Office Suite, particularly Word and Excel.
