Job Summary

ADAPT Community Network is seeking a skilled Community Habilitation Specialist to join our team. As a Community Habilitation Specialist, you will provide on-going training and personal assistance to individuals with disabilities, promoting independence and community inclusion.

Key Responsibilities
  • Provide training and support to individuals with disabilities, focusing on promoting independence and community inclusion.
  • Assess the interests, abilities, strengths, and limitations of the individuals we support.
  • Develop and implement individualized community habilitation plans, incorporating participant desires and program parameters.
  • Provide personal care and daily living skills assistance, adhering to dietary guidelines and participant skill levels.
  • Work collaboratively with supervisors, co-workers, and other service providers to meet program needs.
  • Participate in regular staff meetings and required trainings, maintaining confidentiality and adhering to HIPPA regulations.
Requirements
  • High school diploma required; some college and experience working with people with disabilities preferred.
  • Ability to read, write, and speak English; computer literacy at a level that accommodates training in Electronic Medical Record software.
  • Flexible schedule to meet participant needs; willingness to travel to participant homes or meet in the community.
  • Successful completion of required in-service training and participation in database checks through the Division of Criminal Justice Services and other relevant agencies.
