Job Summary

We are seeking a highly motivated and compassionate Community Habilitation Specialist to join our team at ADAPT Community Network. As a key member of our team, you will play a vital role in empowering individuals with developmental disabilities to live fuller, happier, healthier lives.

Key Responsibilities
  • Provide personalized support and training to individuals with disabilities, promoting independence and community inclusion.
  • Develop and implement individualized habilitation plans, incorporating participant interests, abilities, and goals.
  • Collaborate with participants, families, and other support staff to ensure seamless care and support.
  • Assess and address participant needs, providing guidance and resources as needed.
  • Document participant progress and maintain accurate records.
  • Participate in regular team meetings and training sessions to enhance skills and knowledge.
Requirements
  • High school diploma required; some college and experience working with people with disabilities preferred.
  • Ability to read, write, and speak English.
  • Computer literacy and ability to implement Electronic Medical Record software.
  • Flexible schedule to meet participant needs.
  • Willingness to travel to participant homes or meet in the community.
