Evening Case Manager

The Center for Urban Community Services (CUCS) is a leading human services organization and a major provider of supportive housing and social services.
CUCS is a pioneer in implementing innovative practices, sharing knowledge, and shaping local, state, and national strategies to help individuals who are homeless, low-income, living with mental illness, or have other special needs live successfully in the community.
The Evening Case Manager is responsible for working with clients to help them obtain housing, support their well-being, and enhance their quality of life in the community. This role also involves collaborating with the team and program to ensure effective functioning and meeting contractual obligations, including achieving housing placement goals.
The position requires a strong ability to provide services to a specialized population, excellent computer literacy, and good written and verbal communication skills. Bilingual English/Spanish proficiency is preferred.
Key Responsibilities:
- Provide housing support and case management services to clients
- Collaborate with the team to achieve program goals and objectives
- Develop and implement individualized service plans
- Maintain accurate records and reports
Requirements:
- High school diploma and 2 years of experience (CM 1)
- Bachelor's degree or equivalent and 4 years of relevant work experience (CM 2)
- BSW and 1 year of relevant experience or Bachelor's degree and 2 years of relevant experience or High school diploma and 6 years of relevant experience (CM 3)
Preferred Qualifications:
- Bilingual English/Spanish proficiency
Work Environment:
The Evening Case Manager will work Monday through Friday, 4pm - 12am, as part of a multidisciplinary team.
