About the Role

We are seeking a skilled and compassionate Licensed Vocational Nurse to join our team at Choice Home Health. As a Licensed Vocational Nurse, you will play a vital role in providing high-quality patient care and promoting optimal health outcomes.

Key Responsibilities
  • Provide Direct Patient Care: You will be responsible for delivering skilled nursing care to patients in their homes, including administering medications, wound care, and vital sign monitoring.
  • Assess and Document Patient Progress: You will conduct thorough assessments of patients' health status and document their progress, ensuring accurate and timely communication with healthcare providers.
  • Collaborate with Healthcare Team: You will work closely with patients, families, and healthcare team members to coordinate care, promote quality outcomes, and ensure seamless transitions.
  • Take Verbal Orders and Document: You will receive verbal orders from healthcare providers and document them accurately, adhering to established protocols and guidelines.
  • Provide Education and Support: You will educate patients and caregivers on self-care techniques, medication management, and other essential skills to promote independence and well-being.
Requirements
  • Graduation from an Accredited Nursing Program: You must have graduated from an accredited Licensed Vocational Nursing program and hold a current state license.
  • Clinical Experience: You should have at least one year of clinical experience as a Licensed Vocational Nurse, with a preference for home health experience.
  • Vehicle and Insurance: You must have a reliable vehicle, a valid driver's license, and automobile insurance.
  • Ability to Make Home Visits: You must be able to make home visits and provide skilled nursing care in a variety of settings.
