- Develop and implement individualized care plans, taking into account the unique needs and goals of each client.
- Collaborate with multidisciplinary teams, including physicians, therapists, and other healthcare professionals, to ensure comprehensive care.
- Conduct thorough assessments, identify areas of concern, and develop strategies to address them.
- Provide education and support to clients and their families, promoting healthy coping mechanisms and recovery strategies.
- Current LVN license in good standing.
- Basic computer skills, including proficiency in electronic medical records (EMRs).
- Ability to work effectively in a fast-paced, dynamic environment.
- Strong communication and interpersonal skills, with a focus on empathy and compassion.
- Commitment to the principles of recovery and the importance of a supportive, non-judgmental environment.
