Job Summary:

We are seeking a highly skilled and compassionate Licensed Vocational Nurse to join our team at Choice Home Health. As a Home Health Specialist, you will play a vital role in providing high-quality patient care and promoting independence in the comfort of their own homes.

Key Responsibilities:

  • Provide direct patient care services, including administering medications, wound care, and vital sign monitoring, under the supervision of a registered nurse.
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing personalized care plans to achieve optimal outcomes.
  • Collaborate with healthcare teams to coordinate services, promote quality care, and ensure seamless transitions between care settings.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal care.
  • Document patient progress, including medical history, treatment plans, and outcomes, in accordance with regulatory requirements and company policies.

Requirements:

  • Graduation from an accredited nursing program and current state licensure as a Licensed Vocational/Practical Nurse.
  • At least one year of clinical experience in home health care or a related field, with a strong focus on patient-centered care.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Valid driver's license and reliable transportation, with a clean driving record.
