Job Overview

Position Summary: Deliver comprehensive care to program participants, facilitating their journey towards independence and productivity. Familiarity with computer systems is beneficial.

Key Responsibilities:

  1. Promote activities that enhance each individual's social responsibility, autonomy, dignity, and ongoing development.
  2. Execute objectives aimed at maximizing the independence of each participant, as outlined by the Interdisciplinary Team (IDT), which may include: budgeting, personal grooming, meal planning, utilizing public transport, and engaging with community resources, shopping, and housekeeping.
  3. Successfully complete and demonstrate proficiency in AMAP, SCIP, CPR, and First Aid techniques. Employment is contingent upon passing the required examinations for each course.
  4. Coordinate evacuation procedures and ensure safety during emergencies and fire drills. This includes the ability to navigate fire escapes and assist others, potentially involving both verbal and physical guidance, as well as administering first aid.
  5. Administer medications and/or oversee self-administration as a certified AMAP, including documentation in the Electronic Medication Administration Record (EMAR) system, where applicable.
  6. Manage crisis intervention and adhere to incident reporting protocols.
  7. Monitor and document progress using daily checklists, behavior contracts, quarterly summaries, monthly goal forms, progress notes, family contact logs, and recreation reports.
  8. Exhibit proficiency in entering participant data into the electronic documentation system.
  9. Communicate pertinent information to colleagues and supervisors, both verbally and in writing, including daily logs, incident reports, and medication records.
  10. Engage regularly with participants and the management team to assess goal progress, identify needs, and update program plans. Assist participants in researching and accessing community resources relevant to their goals.
  11. Support participants in planning and participating in evening and weekend recreational activities, including moderate physical activities.
  12. Assist in the organization of daily activities and events.
  13. Provide training in Activities of Daily Living (ADL) and other areas, performing necessary tasks for participants requiring full support, such as personal hygiene, feeding, and mobility assistance.
  14. Attend all scheduled staff meetings, mandatory training sessions, and annual meetings within the program.
  15. Serve as a role model for participants in various settings.
  16. Follow up on aftercare recommendations and accompany participants to appointments as needed.
  17. Maintain confidentiality at all times.
  18. Be adaptable to changes in work schedules and locations based on participant and program needs.
  19. Perform additional duties as assigned.

Qualifications:

A High School diploma or equivalent is required. A valid New York State Driver's License with a minimum of two years of driving experience is necessary, and candidates should be comfortable operating larger vehicles. Experience working with adults with intellectual or developmental disabilities, or other behavioral health challenges, is preferred.
