Position Overview:
Under general supervision, the Healthcare Support Specialist is dedicated to the care and growth of individuals in one of the agency's residential facilities.
This role involves interacting in a way that promotes dignity and encourages learning, ultimately leading to enhanced independence for the residents.
Key Responsibilities:
1. Schedule medical appointments for residents.
2. Organize and/or provide transportation for individuals to their medical appointments.
3. After medical visits, verify if the resident has received a medication order, new prescriptions, or has upcoming appointments.
4. Ensure that medications are properly ordered, documented on the medication administration form, or discharged as necessary.
5. Conduct bi-weekly checks on medications and medical supplies to confirm a five-day supply is available and that medical supplies are adequate to support the medication program.
6. Maintain resident medical records in compliance with agency policies.
7. Document all activities in the electronic medical record system after receiving training.
8. Assist patients to and from examination rooms and prepare them for clinician visits.
9. Communicate relevant clinical findings to the nurse or clinician as needed.
10. Perform Direct Care duties as required.
Compensation:
Minimum Salary: $15.91
Maximum Salary: $16.55
Salary Unit: Hourly
