Job Overview
Horizon CFS LLC seeks a dedicated Direct Service Provider to join our team. As a Direct Service Provider, you will work closely with members, caregivers, and other service providers to ensure effective delivery of services.

Key Responsibilities:
  • Develop and implement service plans that meet the unique needs of each member.
  • Provide support and guidance to caregivers to help them navigate the service system.
  • Collaborate with other service providers to ensure seamless delivery of services.
  • Document progress and outcomes of service delivery.
  • Attend regular meetings with supervisors and other stakeholders to review progress and discuss challenges.

Requirements:
High School Diploma or Equivalent Required, Associates Degree preferred, Bachelor's Degree preferred,
Bilingual Spanish Required,
At least 1 year in related Human Services role, Experience with adolescent/special needs demographic, Knowledge and experience working with children facing mental and behavioral health challenges.
