Care Coordinator for Individuals with Developmental Disabilities

Job Description:
We are seeking a dedicated Care Coordinator to join our team of compassionate professionals. As a Care Coordinator, you will play a vital role in supporting individuals to lead productive lives as members of their local communities as independently as possible.
The responsibilities of this position include:
- Assisting with activities of daily living
- Supporting the development and implementation of goals set in the individuals' ISP
- Ensuring that all safety protocols are implemented within the home and community
- Ensuring all medical protocols and treatments are followed
- Collaborating with clinical and management staff to provide comprehensive care
Estimated Salary Range: $45,000 - $55,000 per year, depending on experience.
Required Skills and Qualifications:
To be successful in this role, you should have:
- A valid driver's license
- Basic computer and internet literacy with the ability to utilize applications like emails, time recording and other programs
- A high school diploma or equivalent is preferred; international students may also apply
- Prior experience in the Human Services field is preferred but not required
- Current First Aid Certification
- CPR certification within one month of hire
- Medication Administration (MAP license) Certification within three months of hire
