Care Coordination Specialist

About the Role:
We are seeking a skilled Care Coordination Specialist to join our team at Mass General Brigham in Medford, Massachusetts.
Estimated Salary: $83,500 - $110,000 per year, depending on experience.
The Care Coordination Specialist will play a vital role in enhancing the delivery of patient care services along the continuum of care. This includes evaluating, predicting, and facilitating the trajectory of patient care through a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation.
Responsibilities:
- Evaluate patients' needs efficiently and expeditiously to ensure timely and effective care delivery.
- Collaborate with healthcare teams to improve patient outcomes and reduce costs.
- Develop and implement care plans that meet individual health care needs.
- Monitor and evaluate patient progress, making adjustments as needed.
- Promote a patient-centered approach to care, ensuring seamless transitions across the care continuum.
Requirements:
- RN Case Manager experience required.
- Licensed RN in the state of Massachusetts.
- Minimum 2 years Med/Surg experience required.
- Case Manager certificate preferred.
- ADN required, but BSN strongly preferred.
- Acute hospital experience preferred.
