About the Role

Job Summary

The Nurse Care Coordinator will be responsible for providing comprehensive care to patients, including assessment, planning, implementation, and evaluation of patient care. The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.

Key Responsibilities
  • Assess patient needs and develop individualized care plans

Requirements

To be successful in this role, you will need:

  • A Bachelor of Science in Nursing degree
  • Current licensure as a Registered Nurse in the Commonwealth of Massachusetts
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a fast-paced environment
