About You

We are seeking a highly motivated and compassionate Registered Nurse (RN) to fill this role. As a RN Care Coordinator, you will be responsible for managing a caseload of patients and ensuring that their care supports desired clinical and financial outcomes.

Your Key Responsibilities

  • Care Facilitation: Coordinate and implement patient plans of care using case management principles.
  • Patient Assessment: Utilize patient assessment, care guidelines, protocols, payer regulations, and response to therapies to assess the episode of illness from pre-admission to post-discharge.
  • Collaboration: Participate in interdisciplinary rounds with nursing staff and physicians to contribute to the plan of care and monitor patient progress.
  • Utilization Management: Collaborate with payers and physicians to ensure appropriateness of admission, continued stay, and reimbursement.
  • Education and Training: Identify learning needs related to case management and work with service leaders to develop educational plans.
  • Continuing Care Needs: Assess continuing care needs in conjunction with other caregivers.
  • Post-Hospital Care: Identify service, treatment, and funding options for post-hospital care and initiate contact with home health agencies and extended care facilities.

Your Skills and Qualifications

  • Graduate of an approved school of nursing with current registration in Massachusetts.
  • Bachelor of Science Degree in Nursing required for newly licensed nurses and external candidates.
  • 1+ years of acute care experience and 1 year of case management experience.
  • Previous experience in a hospital or healthcare setting; bilingual (English/Spanish) preferred.

Salary

$85,000 - $105,000 annually, depending on experience.
