Your Key Responsibilities
  • Develop and implement individualized support plans that promote social participation, skill development, and community engagement.
  • Facilitate group outings and activities designed to foster friendships, build confidence, and encourage independence.
  • Collaborate with colleagues, families, and healthcare professionals to ensure seamless coordination of care and support.
  • Maintain accurate records, track progress, and participate in regular meetings to evaluate program effectiveness.
  • Demonstrate a commitment to customer service excellence, ensuring that every interaction reflects our values of respect, empathy, and compassion.
