Job Summary

Mass General Brigham Healthcare at Home is seeking a skilled and compassionate Registered Nurse (RN) to provide high-quality, patient-centered care in the comfort of patients' homes. As a member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to deliver exceptional care and support.

Key Responsibilities:

  • Provide skilled nursing care to patients in their homes, including assessment, treatment, and education.
  • Develop and implement individualized care plans to meet patients' unique needs and goals.
  • Collaborate with healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Communicate effectively with patients, families, and caregivers to ensure seamless care transitions.
  • Document patient care and progress in a timely and accurate manner.

Requirements:

  • Graduate of an approved School of Nursing.
  • Current Massachusetts license as Registered Nurse.
  • Minimum of one year previous nursing experience in an acute care setting, with prior home care experience preferred.
  • Ability to work independently and as part of an interdisciplinary team.
  • Strong interpersonal and customer service skills, with the ability to work with diverse populations.
  • Ability to communicate effectively in writing, verbally, and electronically.
