The Personal Care Assistant is an integral member of the PACE Interdisciplinary Team. Under the supervision of a licensed nurse, the individual engages in all facets of planning, executing, and assessing personal care and safety for participants both at the Adult Day Health center and within the community.

This is a full-time role, operating Monday through Friday from 8 AM to 4 PM; no evening, weekend, or holiday shifts required.

Key Responsibilities:

  • Provide assistance to participants with various personal care needs at the facility and in the community as directed.
  • Support meal services at the facility, including one-on-one feeding, while adhering to dietary and fluid restrictions and promoting compliance.
  • Assist nursing staff with specimen collection, oxygen administration, and monitoring vital signs.
  • Report any medical concerns or changes in the participant's physical, cognitive, or functional status to the primary nurse.
  • Accompany participants to medical and other relevant appointments as necessary.
  • Carry out all tasks as assigned on a daily basis.
  • Restock supplies and ensure cleanliness in examination rooms, restrooms, kitchens, and storage areas.
  • Sanitize tables, chairs, water coolers, exam tables, and other equipment.
  • Engage in activities, special projects, and therapeutic one-on-one interactions.
  • Measure vital signs and assist in safely transferring participants.
  • Complete required training promptly and attend monthly meetings and training sessions as needed.
  • Adhere to all organizational policies and procedures.
  • Maintain CPR certification as required.
  • Must be able to pass a fit test; the position requires a mask where a proper seal is essential, and facial hair that interferes with the seal is not permitted.
  • Perform additional duties as assigned.

Qualifications:

  • High School diploma or equivalent preferred.
  • HHA certification or equivalent is desirable.
  • At least one year of experience working with the elderly population.
  • Ability to connect with seniors and anticipate their needs effectively.
  • Capable of performing vital sign assessments, lifting participants for transfers, and communicating changes in participant status.
  • Strong verbal and written communication skills.
  • Consistently collaborates in a positive and cooperative manner with the team.
  • Intermediate proficiency in Microsoft Office applications is preferred.
  • Frequent local travel may be required.
