The Personal Care Specialist is an integral member of the PACE Interdisciplinary Team. Under the supervision of a licensed nurse, the incumbent engages in all facets of planning, executing, and assessing personal care and safety for participants both at the facility and within the community.

This is a full-time role, operating Monday through Friday from 8 AM to 4 PM; no evening, weekend, or holiday shifts required.

Location: Lowell, MA

Key Responsibilities:

  • Assist participants with all dimensions of personal care at the facility and in the community as instructed.
  • Support meal assistance, including one-on-one feeding, while adhering to dietary and fluid restrictions.
  • Collaborate with nursing staff for specimen collection, oxygen monitoring, and vital sign checks.
  • Communicate any medical concerns or changes in the participant's physical, cognitive, or functional status to the primary nurse.
  • Provide escort services for medical and other relevant appointments as necessary.
  • Execute daily assignments as outlined.
  • Restock supplies and ensure cleanliness in exam rooms, restrooms, kitchens, and storage areas.
  • Sanitize tables, chairs, and other equipment as required.
  • Assist with activities, special projects, and therapeutic one-on-one interactions.
  • Measure vital signs and safely transfer participants.
  • Complete mandatory training promptly and participate in monthly meetings and training sessions as required.
  • Adhere to all organizational policies and procedures.
  • Maintain CPR certification as mandated.
  • Ability to pass a fit test; position requires a mask where a secure seal is essential, and the incumbent must not have facial hair that interferes with the mask's fit.
  • Perform additional duties as assigned.

Qualifications:

  • High School diploma or equivalent preferred.
  • HHA certification or equivalent is desirable.
  • At least one (1) year of experience working with the elderly population.
  • Ability to connect with seniors and anticipate their needs effectively.
  • Proficient in performing vital signs, lifting participants for transfers, and communicating changes in participant status.
  • Strong verbal and written communication skills.
  • Consistently demonstrates a positive and collaborative approach while working with the team.
  • Intermediate skills in Microsoft Office preferred.
  • Frequent local travel may be required.
