The Lead Physical Nutritional Management Plan Coordinator (PNMPC) operates under the general supervision of the Habilitation Therapy Director or designated personnel, depending on the needs of the State Supported Living Center (SSLC).

The Lead PNMP Coordinator oversees and guides the services provided by direct care staff in areas such as physical and nutritional management, oral and enteral feeding, positioning, mobility, communication, and other related PNMP services.

This role includes conducting competency-based training for staff and mentoring new employees. The Lead Coordinator monitors therapeutic activities, communicates issues, and recommends improvements to Habilitation Therapy staff.

Additionally, the Coordinator ensures the availability, condition, and proper usage of assistive equipment and maintains the appropriate availability and condition of PNMPC instructions and illustrations.

Responsibilities include leading the work of others, distributing assignments, providing coaching to enhance work quality, and acting as a liaison between direct support professionals and Habilitation Therapists.

The Coordinator trains, leads, mentors, and guides staff in implementing planned client therapy programs and in preparing equipment for therapeutic activities, ensuring all equipment is functioning properly.

Participation in PNMP meetings, Nutritional Management Team (NMT) meetings, and Personal Support Team/Health Status Team (PST/HST) meetings is essential.

The Coordinator maintains client records, ensures proper inventory levels, completes data system entries, and oversees the distribution of Physical Nutritional Management Plans (PNMPs) and other documentation under the guidance of Habilitation Therapists.

Responsibilities also include providing new employee orientation and refresher training as needed, as well as overseeing the training of new employees in specific tasks related to lifting, mealtime procedures, and other PNMP activities.

Work hours typically consist of day or evening shifts, with occasional responsibilities for night shift activities.
This position may require driving a state-owned vehicle.

Key Responsibilities:

Regular attendance is required, and the ability to work a specific shift schedule or rotating schedule, extended shifts, and/or overtime may be necessary in accordance with agency leave policies.

The Coordinator leads direct support professionals in daily work assignments, provides on-the-job training and coaching, and communicates issues to the Habilitation Therapy supervisor or designee.

Monitoring all aspects of PNMP-related programming, quality of implementation, and staff compliance is crucial, with findings reported to QIDP, Unit RN, and HT to ensure program effectiveness.

The Coordinator provides orientation, training, and mentoring to new direct support professionals and refresher training to experienced staff.

Oversight of new employee training in lifting, mealtime procedures, and other PNMP-related activities is essential, demonstrating techniques with specific individuals as per their plans.

Competency-based training, coaching, and mentoring are provided to ensure direct care staff are well-prepared and knowledgeable about activities outlined in the PNMPs.

Acting as a liaison between direct support professionals and Therapists is vital to ensure effective communication regarding individual plans, needs, and issues.

Input into the development of PNMPs for assigned individuals is expected to meet the needs agreed upon by the IDT.
Participation as a member of the PNMP Team ensures accuracy in PNMP information. Ensuring appropriate attendance and participation by direct support professionals at assigned meetings and committees is required.

The Coordinator maintains and trains direct support professionals to keep PNMP documentation current and timely, ensuring all staff are informed of changes.

Assisting in implementing planned client care programs and preparing departmental equipment and inventory is part of the role.
Monitoring meals and snacks to ensure safe eating practices is essential.
Providing assistance in transporting individuals during scheduled activities and in emergencies is also required.
The Coordinator performs and oversees accurate data entry and timely maintenance of client records.

Additional duties may include actively participating in or supporting the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation, which may require alternate shift patterns or locations.


Required Knowledge, Skills, and Abilities:
Familiarity with rehabilitative programs for individuals with developmental disabilities.
Understanding of physical nutritional management plans.
Knowledge of oral and enteral feeding, positioning, mobility, communication, and related PNMP services.
Proficiency in using computers for data entry.
Skills in training and coaching others in job responsibilities.
Ability to administer first aid or CPR once trained.
Strong written communication skills with attention to detail.
Ability to follow oral instructions effectively.
Clear verbal communication skills.
Ability to build rapport with colleagues and individuals served.
Competence in collecting, organizing, and entering information into databases.
Ability to assist clients in Habilitation Therapies.
Flexibility in work schedule is essential.
