The Lead Physical Nutritional Management Plan Coordinator (PNMPC) operates under the general oversight of the Habilitation Therapy Director or designated personnel, based on the requirements of the State Supported Living Center (SSLC).

The Lead PNMP Coordinator is responsible for supervising and guiding the services provided to individuals by direct care staff in areas such as physical and nutritional management, oral and enteral feeding, positioning, mobility, communication, and other related PNMP services.

Key responsibilities include conducting competency-based training for staff, mentoring new employees, and monitoring therapeutic activities while communicating any issues and recommendations for enhancements to Habilitation Therapy staff.

You will also oversee the availability, condition, and proper usage of assistive equipment, ensuring that PNMPC instructions and illustrations are in good condition.

This role involves leading and coordinating the work of others, including distributing assignments, providing coaching to enhance work quality, and acting as a liaison between direct support professionals and Habilitation Therapists.

You will train, lead, mentor, and guide others in implementing planned client therapy programs and preparing equipment for therapeutic activities, ensuring that all equipment is functioning properly.

Participation in PNMP meetings, Nutritional Management Team (NMT) meetings, and Personal Support Team/Health Status Team (PST/HST) meetings is also required.

Responsibilities include maintaining client records, ensuring adequate inventory levels, entering data into systems, and distributing Physical Nutritional Management Plans (PNMPs) and other documentation under the guidance of Habilitation Therapists.

You will provide orientation for new employees and refresher training as necessary, overseeing the training of new staff in lifting techniques, mealtime procedures, and other PNMP-related activities.

Work hours typically include day or evening shifts, with occasional responsibilities for night shift activities. This position may also require driving a state-owned vehicle.


Essential Duties:

Regular attendance is crucial, and you may be required to adhere to a specific shift schedule, including rotating shifts, extended hours, or overtime as per agency leave policies, while performing other assigned duties.

You will lead direct support professionals in their daily tasks, providing work assignments, on-the-job training, and coaching, while addressing any issues with the Habilitation Therapy supervisor or designee.

Monitoring all aspects of PNMP-related programming, ensuring quality implementation and staff compliance, and reporting findings to relevant parties is essential for maintaining program effectiveness.

You will provide orientation, training, and mentoring to new direct support professionals, as well as refresher training for experienced staff.

Competency-based training, coaching, and mentoring will be provided to ensure that direct care staff are well-prepared and knowledgeable about the activities outlined in the PNMPs.

Acting as a liaison between direct support professionals and Therapists is crucial for effective communication regarding individual plans, needs, and issues.

You will contribute to the development of PNMPs for assigned individuals, ensuring that the needs agreed upon by the Interdisciplinary Team (IDT) are met.


As a member of the PNMP Team, you will ensure the accuracy of PNMP information and facilitate appropriate attendance and participation by direct support professionals at assigned meetings and committees.

Maintaining and training direct support professionals to keep pertinent documentation in PNMP notebooks up-to-date is vital for keeping all stakeholders informed of any changes.

Assisting in the implementation of planned client care programs and preparing departmental equipment and inventory will also be part of your responsibilities.


You will lead the monitoring of meals and snacks to ensure safe eating practices and provide assistance as needed in transporting individuals during scheduled activities and in emergencies.

Accurate data entry and timely maintenance of client records are essential duties, along with other assigned responsibilities, which may include participating in disaster response and recovery efforts or Continuity of Operations (COOP) activation.


Required Knowledge, Skills, and Abilities:
  • Familiarity with rehabilitative programs for individuals with developmental disabilities.
  • Understanding of physical nutritional management plans.
  • Knowledge of oral and enteral feeding, positioning, mobility, communication, and related PNMP services.
  • Proficiency in using computers for data entry.
  • Skills in training and coaching others to perform job duties.
  • Ability to administer first aid or CPR once trained.
  • Strong written communication skills with attention to detail.
  • Ability to follow oral instructions effectively.

  • Ability to build rapport with colleagues and individuals served.
  • Proficient in collecting, organizing, and entering information into databases.
  • Ability to assist clients in Habilitation Therapies.
