The Lead Support Professional III operates under the guidance of the Habilitation Therapy Director or designated personnel, depending on the needs of the facility.

This role includes overseeing and guiding direct care staff in delivering essential services related to physical and nutritional management, mobility, communication, and other related support services.

You will conduct competency-based training for staff and mentor new employees, ensuring high-quality therapeutic activities and addressing any issues that arise.

Responsibilities also include monitoring the condition and proper usage of assistive equipment, ensuring that all instructions and illustrations for Physical Nutritional Management Plans (PNMPs) are available and in good condition.

In this role, you will lead a team, assign tasks, provide coaching to enhance work quality, and serve as a liaison between direct support professionals and Habilitation Therapists.

Training and mentoring staff in implementing client therapy programs and preparing equipment for therapeutic activities are key components of this position.

Participation in PNMP meetings, Nutritional Management Team meetings, and Personal Support Team meetings is essential.

Maintaining accurate client records, managing inventory levels, and ensuring timely data entry and distribution of PNMPs and related documentation are critical responsibilities.

New employee orientation and refresher training will be part of your duties, along with overseeing the training of new staff in essential procedures.

Work hours typically include day or evening shifts, with some responsibilities for night shift activities.
This position may require the use of a state-owned vehicle.

Key Responsibilities:

Regular attendance is expected, with potential requirements for specific shift schedules, rotating shifts, or overtime as needed.

Lead and support direct care professionals in their daily tasks, providing on-the-job training and addressing any issues with the Habilitation Therapy supervisor.

Monitor all aspects of PNMP programming, ensuring quality implementation and compliance, and report findings to relevant team members.

Provide orientation and training to new staff, ensuring they are well-prepared for their roles.

Oversee training in critical topics such as lifting and mealtime procedures, demonstrating with specific individuals as needed.

Deliver competency-based training and coaching to ensure staff are knowledgeable about PNMP activities and that plans are executed as intended.

Facilitate communication between direct support professionals and Therapists to ensure clarity regarding individual plans and needs.

Contribute to the development of PNMPs tailored to meet the needs of assigned individuals.
Serve as an active member of the PNMP Team, ensuring accurate information is maintained.

Ensure direct support professionals attend and participate in assigned meetings and committees.

Maintain timely documentation in PNMP notebooks to keep all staff informed of changes.

Assist in implementing planned client care programs and prepare departmental equipment and inventory as necessary.
Oversee meal monitoring to ensure safe eating practices.
Provide assistance in transporting individuals for scheduled activities and during emergencies.
Ensure accurate data entry and timely maintenance of client records.

Additional duties may include participating in disaster response and recovery efforts or Continuity of Operations (COOP) activation, which may require alternate shift patterns or locations.


Required Knowledge, Skills, and Abilities:
Understanding of rehabilitative programs for individuals with developmental disabilities.
Familiarity with physical nutritional management plans.
Knowledge of oral and enteral eating, positioning, mobility, communication, and related services.
Proficiency in computer data entry.
Ability to train and coach others effectively.
Capability to administer first aid or CPR after appropriate training.
