Job Summary

We are seeking a highly skilled and compassionate Direct Support Professional to join our team at Hearts and Hands of Care, Inc. As a Direct Support Professional, you will play a vital role in providing ongoing services to clients and individuals with needs in their community or in their home.

Key Responsibilities
  • Provide high-quality support services to clients, ensuring their safety and well-being at all times.
  • Develop and implement individualized Service Plans, working closely with clients, families, and team members to achieve their goals and objectives.
  • Document daily activities, progress, and challenges, maintaining accurate and comprehensive records.
  • Collaborate with the Child and Family Team to promote community-based care and support the most appropriate and least restrictive settings.
  • Assist clients with daily living activities, such as personal care, meal preparation, and household management.
  • Provide emotional support and companionship to clients, promoting their independence and self-esteem.
  • Communicate effectively with clients, families, and team members, maintaining open and respectful relationships.
  • Participate in ongoing training and professional development to enhance skills and knowledge.
  • Adhere to agency policies and procedures, maintaining confidentiality and upholding professional standards.
Requirements
  • High school diploma or equivalent required; degree in a related field preferred.
  • At least 18 years of age; must meet all requirements for the position as set out in relevant laws and regulations.
  • Must pass a Criminal Background Check; may not have been denied a healthcare provider license or certification for a reason related to patient services.
  • Ability to work independently and as part of a team; excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Reliable and dependable; ability to work varied schedules, including evenings and weekends.
