We are seeking a skilled
Family Support Coordinator
who is passionate about making a positive difference in the lives of families.

Work Schedule:
This role requires availability from Monday to Saturday with flexible hours.

Role Purpose:
The Family Support Coordinator will deliver comprehensive case management services for a minimum of 30 active cases involving families with children aged from birth to 6 years. This position entails:
  • Maintaining case ownership to coordinate necessary services and support for each child and family member, focusing on health, wellness, and safety.
  • Managing and monitoring the support plan, assessing family dynamics, and adjusting services as needed.

Key Responsibilities:
  • Plan and execute early childhood events at various centers to engage families and facilitate referrals for developmental screenings.
  • Conduct home visits to assess family needs and ensure successful access to essential services.
  • Collaborate with parents and caregivers to create tailored family support plans.
  • Provide timely referrals to appropriate services.
  • Work alongside families to strengthen parent-child relationships and enhance their support networks.
  • Maintain consistent communication with service providers and document progress effectively.
  • Deliver community education through presentations and outreach.
  • Organize and prioritize tasks to meet established deadlines.
  • Develop a thorough understanding of community resources, eligibility requirements, and emergency procedures.
  • Collaborate with volunteers and community partners.
  • Stay informed about local child-serving systems and resources.
  • Accurately document all case activities and maintain organized case files.
  • Foster relationships with local programs to provide families with relevant information and resources.
  • Attend all required meetings, trainings, and workshops.
  • Work flexible hours, including evenings and weekends as necessary.
  • Perform additional duties as assigned.

Additional Information:
The ideal candidate will demonstrate cultural sensitivity and an understanding of the socioeconomic characteristics of the service population. Familiarity with the Protective Factors framework and Family Support Principles is essential. Bilingual skills may be beneficial for communication purposes.

Physical Requirements:
The role demands high energy levels, adaptability to varying hours, and the ability to travel as needed.

Qualifications:
A Bachelor's Degree in Social Work, Early Childhood Education, or a related field is required.

Experience:
Experience in managing casework for families with children aged birth to 6 years is essential. Bilingual English/Spanish is preferred.

Skills:
The coordinator should possess a strong knowledge base and the ability to establish connections with community resources to effectively link families with their identified needs. Adaptability and the ability to manage multiple tasks are crucial.

Accountability:
This position reports to the Service Coordinator Supervisor and requires teamwork with colleagues and adherence to agency policies and procedures, ensuring confidentiality of all client information.
