Job Summary

We are seeking a skilled Medical Social Worker to join our team at Northwell Health. As a Medical Social Worker, you will provide individual and group work services to patients and their families in various healthcare settings.

Key Responsibilities
  • Provide culturally competent direct service to patients and their families using a variety of social work modalities.
  • Utilize high-risk criteria and/or consultation referrals to identify patients in need of or who could benefit from social work assessment and intervention.
  • Advocate on behalf of the individual to ensure maximum access to healthcare, assess barriers, and offer solutions.
  • Provide interdisciplinary team with assessment findings and suggest potential impact on overall care of patient.
  • Provide psychoeducation, counseling, and crisis intervention to patients and their families.
  • Formulate and implement treatment and discharge plans that are safe and timely and in line with the expectations of the site and promote patient choice.
  • Initiate communication with post-acute providers and payers as needed.
  • Incorporate knowledge of community resources and entitlement programs in all planning.
  • Maintain appropriate professional patient care documentation as defined by site policies and procedures.
  • Represent department and profession in various hospital/site-based committees and work groups.
Requirements
  • Master of Social Work required.
  • Current License to practice as a Master Social Worker, or Current License to practice as a Clinical Social Worker in New York State required, plus specialized certifications as needed.
  • Experience in discharge planning and working in a hospital setting, extremely preferred.
  • 1-3 years of relevant experience, required.
