Job Summary

The School-Based Health Social Worker provides Behavioral Health Services to patients within the School-Based Health Department managed by NORTH COUNTRY FAMILY HEALTH CENTER. This role involves evaluating and treating patients with behavioral health diagnoses, as well as providing input and support for other clinical staff involved in patient care.

Key Responsibilities
  • Evaluate and treat patients with behavioral health diagnoses
  • Provide input and support for other clinical staff involved in patient care
  • Travel to school-based locations as required
Qualifications

The Social Worker will have a minimum degree of New York State:

  • Licensed Master Social Worker (LCSW-R)
  • Licensed Master Health Counselor (LMHC)

This position requires excellent verbal, listening, and documentation skills, as well as complete and accurate record keeping for quality of care, treatment planning, and billing of behavioral health services.
