Job Title: LTSS Service Coordinator Clinician

About the Role:

This is a field position that requires 4-5 days of work per week. You will be working under the direction of a Registered Nurse (RN) and will be responsible for performing activities within the scope of your licensure. Your primary goal will be to assist the RN in identifying members who require high-risk interventions and to provide clinical data as directed.

Responsibilities:
  • Assist the RN in identifying members who require high-risk interventions and provide clinical data as directed.
  • Obtain clinical data as directed by the RN and assist in identifying members who would benefit from alternative levels of care or other waiver programs.
  • Provide all information collected to the RN, who will verify and interpret the information, conduct additional assessments as necessary, and develop, monitor, evaluate, and revise the member's care plan to meet their needs.
  • Participate in coordinating care for members with chronic illnesses, co-morbidities, and/or disabilities as directed by the RN, and in conjunction with the RN, member, and healthcare team, to ensure cost-effective and efficient utilization of health benefits.
Requirements:
  • LPN/LVN, LSW, LCSW, or LMSW or license other than RN in accordance with applicable state law and Nursing Diploma or AS in Nursing or a related field.
  • Minimum of 2 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role.
  • Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license other than RN (as allowed by state law) in applicable state(s) required.
Preferred Skills:
  • Proficiency in Microsoft Office (Excel, Word, Outlook, etc.) strongly preferred.
  • Case management experience strongly preferred.
  • Home health experience strongly preferred.
  • Medicaid experience strongly preferred.
  • Discharge planning experience strongly preferred.
  • MA/MS in Health/Nursing preferred.
