POSITION SUMMARY: The Client Support Specialist is tasked with evaluating the needs and capabilities of clients while guiding them towards achieving their personal objectives. This role involves conducting annual evaluations, reassessments, and developing tailored service plans. The Specialist plays a crucial role in facilitating clients' integration into their communities and establishing connections to essential resources such as healthcare, vocational services, benefits, legal support, housing, and family assistance.
KEY RESPONSIBILITIES:
- Maintain and update client medical records in accordance with regulatory standards and organizational policies.
- Conduct annual evaluations, reassessments, and create individualized service plans.
- Act on behalf of clients with integrity and respect, ensuring open communication with family members and other stakeholders.
- Assist clients in obtaining and retaining their benefits.
- Monitor clients' progress towards their goals.
- Provide crisis intervention support as necessary.
- Facilitate housing placements for clients when required.
- Utilize creativity, judgment, and collaboration to address client issues effectively.
- Conduct referral, intake, and orientation processes for new clients.
- Accompany clients to appointments as needed.
- Foster positive and engaging interactions with clients.
- Ensure timely and accurate documentation of treatment plans, contact notes, and other required records.
- Meet productivity expectations as outlined by the Program Manager.
- Adhere to the policies and procedures of People Encouraging People, ensuring compliance with relevant regulatory standards.
QUALIFICATIONS:
- Associate's Degree in mental health or a related discipline.
- At least one year of experience in mental health or a related field.
- Strong organizational skills with the ability to prioritize effectively.
- Flexibility to accommodate client needs.
- Ability to thrive in a collaborative team environment.
