Position Overview:

Constellation Health Service is seeking a dedicated and empathetic Palliative Care Nurse Practitioner (NP) to join our esteemed palliative and hospice program. This position entails delivering care in patients' residences as well as various facilities within the community.

Key Responsibilities:

  • Conduct thorough assessments and collaborate on care plans with all relevant disciplines, addressing the needs of patients, families, and primary caregivers.
  • Monitor, assess, and communicate any changes in the patient's physical and emotional condition to the nursing supervisor and attending physician.
  • Administer skilled nursing care, including medication and treatment as per the physician's directives for patients in their home environment.
  • Engage in weekly Interdisciplinary Team Meetings, sharing updates on the patient's physical, emotional, psychosocial, and spiritual well-being.
  • Ensure all documentation adheres to agency standards and is processed accurately.
  • Utilize a computer-based documentation system to submit all necessary clinical records, time logs, and other required information punctually.
  • Act as a liaison with healthcare institutions to facilitate continuity of care as patients transition between different levels of care.
  • Participate in community initiatives as a representative of the agency.
  • Perform additional duties as assigned.

Qualifications:

  • Master's degree, post-master's certificate, or doctoral degree in nursing from an accredited institution.
  • Valid nursing license in the state where the hospice program operates.
  • Current National Board Certification as a Clinical Nurse Specialist (CNS) or Nurse Practitioner (NP).
  • Preferred experience in hospice or palliative care, clinical oncology, geriatrics, intensive care, or community and family health.
  • Palliative care certification (NBCHPN) or hospice certification (ACHPN) is preferred.
  • Completion of initial credentialing and re-credentialing every two years is required.
