Job Summary

Constellation Health Services is seeking a compassionate and empathetic Palliative Care Nurse Practitioner (NP) to join our team. As a Palliative Care Nurse Practitioner, you will provide high-quality patient care in a home setting, working collaboratively with our interdisciplinary team to coordinate care and ensure the best possible outcomes for our patients.

Key Responsibilities
  • Assessment and Coordination of Care: Perform comprehensive assessments of patients' physical and emotional needs, and coordinate care with all disciplines to ensure seamless delivery of services.
  • Skilled Nursing Care: Provide skilled nursing care, including administration of medications and treatments as prescribed, to patients in their home setting.
  • Interdisciplinary Collaboration: Participate in weekly Interdisciplinary Team Meetings to provide updates on patients' physical, emotional, psychosocial, and spiritual status.
  • Documentation and Communication: Maintain accurate and timely documentation of patient care, and communicate effectively with patients, families, and healthcare teams.
  • Liaison and Community Engagement: Act as a liaison with institutions to promote continuity of care, and participate in community activities to promote the agency's mission and values.
Requirements
  • Education: Master's degree, post-masters certificate, or doctoral degree in nursing from a university accredited by the Commission on Collegiate Nursing Education (CCNE) or by the National League for Nursing Accrediting Commission (NLNAC).
  • Licensure: Current license to practice nursing in the state of New Hampshire and/or Maine where the hospice program operates.
  • Certification: Current National Board Certification from ANCC or AANP as a Clinical Nurse Specialist (CNS) or Nurse Practitioner (NP).
  • Experience: Experience in hospice or palliative care, clinical oncology, geriatrics, intensive care, or community and family health preferred.
  • Certifications: Palliative care certification (NBCHPN) or hospice certification (ACHPN) preferred.
