Job Summary

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. As a key member of our clinical team, you will conduct comprehensive health assessments in the comfort of patients' homes, providing critical insights into their overall health and well-being.

Key Responsibilities
  • Conduct thorough adult and geriatric assessments, medical history reviews, and diagnosis and treatment planning
  • Collaborate with physicians, case managers, and other healthcare professionals to ensure seamless care coordination
  • Provide education and support to patients and their families, promoting healthy behaviors and disease management
  • Work in a variety of settings, including home visits, skilled facilities, and telehealth
Requirements
  • Master's degree in Nursing or Physician Assistant studies, or equivalent experience and licensure
  • Current RN and NP or PA licensure, with prescription authority or ability to obtain
  • Board certification in a Matrix-approved specialty
  • Minimum 1 year experience as a Nurse Practitioner or Physician Assistant, with strong computer skills and EMR software familiarity
  • Excellent communication and interpersonal skills, with a positive and flexible attitude
