We are seeking a skilled Nurse Care Coordinator to join our team at Euclid Hall. As a key member of our Social Services Team, you will provide a range of health coordination services to our residents, enabling them to maintain safe and permanent affordable housing with as high a degree of independence as possible.
Responsibilities:- Assess residents' physical and mental health needs, identifying any existing issues that require attention and monitoring changes over time.
- Ensure high-risk residents receive extra assistance with health education, care coordination, and follow-up, including scheduling and transportation.
- Track residents' healthcare compliance and schedule care coordination conferences as needed to incentivize their compliance and improve their health.
- Collaborate with our on-site Federally Qualified Health Center to cultivate a mutual relationship of patient care coordination.
- Support residents in obtaining and maintaining ongoing medical care by providing education on medication and medical treatments as needed.
- Provide workshops, training, and activities to offer residents access to vital health, nutrition, and wellness information, including Wellness Self-Management.
- Coach our site-based supportive services team members on best practices in healthcare coordination.
- Coordinate and collaborate with a multidisciplinary internal and external care team to address and resolve resident medical needs.
- Train staff to ensure medication information is available for scheduled medical appointments and emergency interventions.
- Provide emergency assessments and recommend emergency medical care as needed.
- Assist residents and staff with hospital and nursing home discharge planning.
- Refer residents to external services, including CCHAs and home care agencies.
- RN or LPN with 1-3 years of experience in the profession.
- Current NYS license required.
- Experience working with individuals who have serious mental illness is strongly preferred.
- Experience training/supporting care team colleagues on effective client health coordination.
- Experience providing individual and group wellness education to clients.
- Experience working with the homeless and/or the elderly is preferred.
- Bilingual (English/Spanish) language ability is helpful.
