Regal Heights Rehabilitation & Nursing Center is a renowned nursing facility dedicated to providing exceptional care for its residents. We are seeking an experienced Care Coordinator to join our team, responsible for ensuring the social and emotional well-being of our elderly residents.

Estimated Salary: $55,000 - $65,000 per year.

**Job Description:**

As a Care Coordinator at Regal Heights, you will play a vital role in maintaining and improving the residents' ability to control their everyday physical needs and mental/psychosocial needs. Your primary responsibilities will include:

* Initiating appropriate interventions for residents experiencing mental or behavioral health issues
* Identifying individual social and emotional needs
* Participating in the planning of the resident's admission, community re-entry, or transfer to another facility
* Collaborating with the finance team to ensure the resident is educated and the proper documents are in place for the duration of their stay
* Facilitating discharge planning and arranging necessary DME or home care to ensure the resident's safe return to their community

**Required Skills and Qualifications:**

* BSW or MSW in a related field
* Nursing Home or Hospital experience (internship sufficient)
* Bi-lingual English/Spanish (preferred but not required)
