Job Summary

We are seeking a skilled and compassionate Nurse Coordinator to join our team at Northwell Health. As a key member of our certified agency team, you will play a vital role in managing patient care related to services provided in the home.

Key Responsibilities
  • Conduct home visits to assess patient needs and develop comprehensive care plans
  • Collaborate with patients, families, and physicians to ensure seamless care coordination
  • Provide skilled nursing care, including wound care, injections, and medication management
  • Orient and supervise home health aide personnel to ensure regulatory compliance
  • Evaluate patient progress and adjust care plans as needed
Requirements
  • Graduate of an approved school of professional nursing
  • Currently licensed RN in NYS
  • Home care experience preferred
  • Excellent verbal and written communication skills
Physical Requirements
  • Ability to operate medical equipment
  • Ability to perform physical and psychosocial assessments of clients and families
  • Ability to travel and climb stairs
