Job Summary:

The Institute for Community Living, Inc. is seeking a skilled Registered Nurse Assessor to join our team. The successful candidate will be responsible for assessing the physical health needs of residents in impacted Adult Homes and developing care plans to support their transition into independent living arrangements.

Key Responsibilities:

Coordinate with the Social Worker Assessor and multi-disciplinary team members to streamline the assessment and placement process

Provide comprehensive nursing assessments of the health status of residents and develop nursing diagnoses and care plans

Implement nursing strategies or interventions according to the established care plan

Evaluate resident's and caregiver's responses to care plan and advise Care Manager as appropriate

Requirements:

NYS license and valid, current registration as a Registered Professional Nurse

At least 5 years' experience providing direct services to individuals with mental illness

Highly skilled at performing physical health assessments and developing and monitoring appropriate plans of care
