Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our team at TemPositions Health Care. As a Registered Nurse, you will be responsible for providing high-quality nursing care to patients in their homes, managing and coordinating the delivery of multidisciplinary health care services, and collaborating with a team of healthcare professionals to ensure optimal patient outcomes.

Key Responsibilities
  • Assess the physical, social, and psychological needs of patients and develop individualized plans of care.
  • Communicate and consult with patients' physicians and other healthcare professionals to ensure seamless care coordination.
  • Manage and coordinate all aspects of assigned caseloads, including patient assessments, care planning, and discharge planning.
  • Establish and implement plans of care in collaboration with multidisciplinary healthcare teams, including physicians, therapists, and other healthcare professionals.
  • Ensure that services follow established plans of care through ongoing assessment, planning, coordination, counseling, teaching, and supervision of care delivery.
  • Oversee and supervise the care provided to patients by Licensed Practical Nurses and Paraprofessionals through regular home visits and compliance with regulatory requirements.
  • Assess patients' conditions, monitor the effect of care plans on patient health status, and inform physicians and other team members of changes in patient needs and update plans of treatment as needed.
  • Initiate and maintain written and verbal communications with team members and management to ensure optimal patient care.
  • Assist patients with activities of daily living, including positioning, moving, transferring, and lifting patients of varying weights and physical conditions, with and without assistance from family members and/or paraprofessionals.
  • Manage, plan, coordinate, and execute patients' safe discharge from service, ensuring that physician orders, discharge summaries, and communication to team members, patients, and family members adhere to regulatory requirements.
  • Follow work procedures in relation to established workflows and perform other related duties as needed.
Qualifications
  • Licensure: Current registration to practice as a registered professional nurse in New York State required.
  • Minimum of one year of clinical nursing experience.
