Job Title: Coordinator of Care RN

Join MJHS Home Care as a Coordinator of Care RN and play a vital role in delivering exceptional patient care in the comfort of their own homes.

About the Role:

As a Coordinator of Care RN, you will be responsible for providing comprehensive care to patients recovering from illness, injury, or surgery. You will work closely with patients, families, and healthcare teams to develop and implement personalized care plans that meet their unique needs.

Key Responsibilities:
  • Conduct thorough patient assessments to identify care needs and develop individualized care plans
  • Coordinate and implement care plans, including medication management and patient education
  • Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making
  • Monitor patient progress and adjust care plans as needed to ensure optimal outcomes
Requirements:
  • Current NYS RN license and registration
  • Bachelor's degree in nursing preferred; Associate's degree required
  • Minimum of one year acute medical-surgical nursing experience
  • Certified Home Health Agency (CHHA) experience preferred
  • Bilingual English and Spanish or Mandarin or Cantonese preferred
