About the Role

This is an exciting opportunity to join our team as a Family Service Care Coordinator. As a key member of our Family Care Community Partnership Program, you will play a vital role in supporting the success of families living in Rhode Island.

Key Responsibilities
  • Assist and educate families involved in transition to Family Care Community Partnership (FCCP) through family advocacy, education of the service planning process, and implementing treatment plans focused on family-centered practices.
  • Help families revise the Family Care Plan as needed and ensure all appropriate people are included, adhering to all HIPPA and confidentiality guidelines.
  • Collect and analyze data to serve evaluation and assessment of the Family Care Plan.
  • Provide ongoing support to families through empowerment and motivation.
  • Work with parents to improve child management skills through modeling, education, support, and encouraging proper parental coping skills.
  • Coordinate services with schools, police, healthcare providers, court systems, and other service providers.
Requirements and Qualifications
  • Bachelor's degree in social work or a related field is preferred.
  • Experience in child welfare, parent education, home-based services, and/or crisis intervention is required.
  • Knowledge of child and adolescent development, family dynamics, substance abuse, home management, and parenting skills is preferred.
  • Bilingual in Spanish is preferred.
  • Strong organizational, critical thinking, and time-management skills are essential.
  • Basic computer skills and knowledge of Microsoft Office 365 are required.
  • Ability to communicate and effectively interact with a diverse population.
  • Collaboration and teamwork are essential.
  • A valid driver's license, proof of auto insurance, and car registration are required.
  • Authorization for a motor vehicle records check is required.
  • Background checks and medical clearances are required for the position.
Physical Requirements

This position requires time sitting, standing, walking, driving, carrying, and lifting up to 25lbs. The role also requires operating desktop, laptop, and/or cell phone, and communicating with colleagues.

Travel Requirements

Site location: Middletown. Statewide travel is required.
