Job Summary

Horizon Services, Inc. is seeking a highly skilled and compassionate SUD Counselor II to join our team at Palm Avenue Detox. As a key member of our clinical staff, you will play a vital role in creating a welcoming atmosphere for incoming clients and ensuring their successful transition into continued treatment.

Key Responsibilities
  • Collaborate with the Program Manager to develop and implement individualized treatment plans
  • Conduct intakes, assessments, and progress monitoring to ensure clients meet daily census requirements
  • Communicate effectively with Clinical Supervisors and Program Managers to ensure services align with department policy
  • Provide compassionate and empathetic care to all program participants, utilizing evidence-based practices
  • Perform ASAM screenings and assessments, as well as vitals sign patient self-monitoring tasks
  • Develop and maintain positive working relationships with staff members and community organizations
  • Coordinate referrals and discharge planning to ensure seamless transitions
  • Maintain accurate records and data entry in our Electronic Health Record System
Requirements
  • Registration or Certification with a Substance Use state certifying body in California (CAADE/CCAPP/CADTP)
  • Excellent computer skills and knowledge of data capture and data input into Electronic Health Records
  • Knowledge of alcohol and drug addiction, as well as social model detox concepts
  • Experience working with diverse populations and cultures, including those with co-occurring disorders and disabilities
