We are currently seeking a full-time Intake Coordinator to join our team in the Woburn, MA branch. This position operates Monday through Friday from 8 AM to 5 PM, with a rotation for weekend duties. Initial training will be conducted on-site, with potential for partial remote work following successful training completion.
Position Summary:
This entry-level role is essential for the comprehensive coordination of care for patients receiving home infusion therapy. A Senior Intake Coordinator will serve as a mentor to facilitate your development.
The Intake Coordinator position features a structured Career Ladder. Candidates demonstrating outstanding performance, commitment to personal growth, and strong collaborative relationships may advance to higher roles:
- Senior Intake Coordinator: Expected promotion within 2-5 years for those meeting defined goals and demonstrating engagement.
- Lead Intake Coordinator: Involves leadership responsibilities within the department and engagement with company and hospital partners.

Key Responsibilities:
- Compile initial Patient Profiles for therapy and distribute to relevant healthcare team members.
- Manage incoming calls for the Intake Workgroup, ensuring all communications are documented appropriately.
- Effectively communicate with pharmacy, reimbursement, and delivery teams regarding patient referrals and ongoing issues.
- Collaborate with the Liaison/Care Coordination team to facilitate efficient patient discharge.
- Document all patient-related information and communications in clinical records.
- Update patient supply requirements and ensure accurate processing of delivery requests.
- Correct any discrepancies in patient files as identified through daily reports.
- Adhere to company policies and procedures in all business activities.
- Develop understanding of insurance coverage with guidance from senior staff.
