Job Overview

MCM & Associates LLC is seeking a motivated and skilled Clinical Liaison specializing in hospice care to enhance our operations and outreach efforts.

The primary role of a Clinical Liaison is to cultivate and sustain connections with referral sources within healthcare facilities. A successful Clinical Liaison is an expert in hospice services, utilizing their clinical knowledge to educate patients and their families while ensuring a seamless transition to hospice care.

Key Responsibilities:

  • Enhance community and referral perceptions of MCM & Associates LLC as a premier provider of hospice services.
  • Conduct regular outreach to secure new referrals, increase patient census, and achieve targets with referring physicians, healthcare facilities, case managers, discharge planners, and social workers.
  • Facilitate initial meetings with patients and families to provide an overview of hospice services and obtain necessary consents.
  • Coordinate the referral process with the admissions and intake team to ensure efficient patient onboarding.
  • Establish and nurture positive relationships with current and prospective referral sources.
  • Contribute to strategic planning by identifying opportunities for enhanced services to address unmet community needs.
  • Maintain a thorough understanding of community resources and assist referral sources in accessing them when hospice services are not applicable.
  • Perform additional duties as required.

Qualifications:

  • Bachelor's degree in nursing or healthcare preferred.
  • Current LVN license is required.
  • At least one year of experience in sales or marketing within the healthcare sector.
  • One to two years of experience in hospice or palliative care marketing.

Job Type: Full-time

Compensation: $40.00 per hour

Experience Required:

  • Sales/Marketing: 1 year (Required)

Preferred Language: Spanish

License/Certification: LVN (Required)
