Patient Care Coordinator

Frye Regional Medical Center is seeking a skilled Nurse Care Specialist to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and support to our patients and their families.
Key Responsibilities- Provide direct patient care, including administering medications, collecting specimens, and performing clinical treatments.
- Document patient care and progress, ensuring accurate and timely communication with the care team.
- Monitor and record patient condition, communicating with the care team and physicians as needed.
- Assist patients with daily living activities, including personal hygiene, elimination, nutrition, and ambulation.
- Collaborate with the care team to coordinate patient care, including patient transfer, discharge, referral, and spiritual/psychosocial support needs.
- Evaluate patient learning needs and provide education and support to patients and their families.
- Current state RN license
- Bachelor's degree from an accredited nursing school
- Basic Life Support certification within 30 days of hire
- ACLS and PALS certification within six months of hire
- One year of experience preferred
- ASLS certification preferred
