Nurse Manager

VNA Health Group is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health RN Case Manager. This is a full-time opportunity to provide high-quality care to patients in their homes, working closely with our interdisciplinary team to ensure seamless care coordination.
Responsibilities:- Assess patients' needs and develop individualized care plans to meet their unique requirements.
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed.
- Collaborate with healthcare providers, family members, and caregivers to ensure comprehensive care and support.
- Conduct regular home visits to monitor patients' progress, address concerns, and make adjustments to care plans as needed.
- Document patient information, progress, and outcomes in a timely and accurate manner.
- Current RN licensure in the State of Ohio.
- 1-2 years of experience in home health care or a related field.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
- Ability to work independently and as part of a team, with a focus on patient-centered care.
- Valid driver's license and CPR certification.
