Job Title: Home Health RN Case Manager

VNA Health Group is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health RN Case Manager. This is a full-time opportunity to provide high-quality patient care and make a meaningful difference in the lives of our patients.

Job Summary:

The RN Case Manager will be responsible for providing direct or indirect skilled comprehensive nursing care to a specified age and diagnostic group. This includes assessing patient needs, developing and implementing individualized care plans, and collaborating with healthcare providers to ensure seamless care transitions.

Responsibilities:
  • Utilize principles of the nursing process to assist in data collection, planning, and evaluation of nursing care.
  • Demonstrate nursing knowledge and skills in the implementation of prevention, therapeutic, and rehabilitative nursing care.
  • Communicate and collaborate effectively with healthcare providers, patients, and families.
  • Maintain awareness of patient comfort and safety needs.
  • Respond to life-saving situations based on nursing standards, policies, and protocols.
Qualifications:
  • Experience in home health care.
  • Experience working as part of a team and working effectively with diverse cultural, social, and ethnic backgrounds.
  • Ability to work in a complex institutional structure requiring significant collaboration.
  • Strong planning and organizational skills.
  • Ability to work well under pressure and maintain attention to detail.
Requirements:
  • 1-2 years' experience in Med Surg or hospital setting.
  • Completion of an accredited RN education program with a diploma, Associate's or Bachelor's degree.
  • Valid driver's license and CPR certification.
