Responsibilities:- Provide comprehensive nursing care to patients in their homes, focusing on prevention, therapeutic, and rehabilitative care.
- Conduct thorough assessments, develop care plans, and implement interventions to meet patients' needs.
- Collaborate with healthcare teams, patients, and families to ensure seamless care coordination.
- Utilize effective communication and interviewing techniques to gather data and set limits as needed.
- Maintain awareness of patients' comfort and safety needs, responding promptly to any concerns.
- 1-2 years of experience in Med Surg or hospital setting.
- Completion of an accredited RN education program with a diploma, Associate's or Bachelor's degree. Bachelor's degree in Nursing preferred.
- Registered Nurse, licensed in the State of Ohio.
- Experience in home health care and 1 year experience in medical-surgical nursing. In addition to Acute Care experience.
- Valid driver's license and CPR certification required.
