Job Title: Home Health RN Case Manager

VNA Health Group is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health RN Case Manager. This is a full-time opportunity to provide high-quality patient care and make a meaningful difference in the lives of our patients.

Job Summary:

The Home Health RN Case Manager will be responsible for providing comprehensive nursing care to patients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans. This role requires strong communication and organizational skills, as well as the ability to work independently and as part of a team.

Responsibilities:
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted care plans.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions and optimal patient outcomes.
  • Provide education and support to patients and families on disease management, medication adherence, and healthy lifestyle habits.
  • Monitor patients' progress, adjusting care plans as needed to ensure optimal outcomes.
  • Document all patient interactions, assessments, and care plans in a timely and accurate manner.
Requirements:
  • Current RN licensure in the State of Ohio.
  • 1-2 years of experience in home health care or a related field.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare providers.
  • Ability to work independently and as part of a team, with a strong attention to detail and organizational skills.
  • Valid driver's license and CPR certification.
