VNA Health Group is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health RN Case Manager. This is a full-time position that requires a strong commitment to delivering high-quality patient care in a home health setting.
Job Summary:The Home Health RN Case Manager will be responsible for providing comprehensive nursing care to patients in their homes, working closely with healthcare teams to develop and implement individualized care plans. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Responsibilities:- Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted care plans.
- Collaborate with healthcare teams to coordinate care, including ordering medications, scheduling appointments, and communicating with patients and families.
- Provide direct patient care, including administering medications, performing wound care, and providing education on self-care and disease management.
- Monitor patients' progress, adjusting care plans as needed to ensure optimal outcomes.
- Communicate effectively with patients, families, and healthcare teams, ensuring seamless care transitions and minimizing hospital readmissions.
- Current RN licensure in the State of Ohio.
- 1-2 years of experience in home health care or a related field.
- Strong knowledge of nursing principles, practices, and standards.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Valid driver's license and CPR certification.
