Job Title: Home Health RN Case Manager

VNA Health Group is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health RN Case Manager. This is a full-time position that requires a strong commitment to delivering high-quality patient care in a home health setting.

Job Summary:

The Home Health RN Case Manager will be responsible for providing comprehensive nursing care to patients in their homes, working closely with the healthcare team to develop and implement individualized care plans. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities:
  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement individualized care plans in collaboration with the healthcare team
  • Provide direct patient care, including wound care, medication management, and education
  • Collaborate with other healthcare professionals to ensure seamless care transitions
  • Document patient progress and outcomes in a timely and accurate manner
Requirements:
  • Current RN licensure in the State of Ohio
  • 1-2 years of experience in home health care or medical-surgical nursing
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Valid driver's license and CPR certification
