Job Title: Home Health RN Case Manager

VNA Health Group is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Home Health RN Case Manager. This is a full-time position that requires strong nursing skills, excellent communication, and a passion for delivering high-quality patient care.

Job Summary:

The successful candidate will be responsible for providing comprehensive nursing care to patients in their homes, working closely with healthcare teams to develop and implement individualized care plans. This role requires strong organizational and time management skills, as well as the ability to work independently with minimal supervision.

Responsibilities:
  • Conduct thorough assessments of patients' physical and emotional needs, identifying areas for improvement and developing targeted care plans.
  • Collaborate with healthcare teams to coordinate patient care, including medication management, wound care, and other interventions.
  • Provide education and support to patients and their families, promoting self-care and independence.
  • Document patient progress and outcomes, maintaining accurate and detailed records.
  • Participate in quality improvement initiatives, contributing to the development and implementation of best practices.
Requirements:
  • Current RN licensure in the State of Ohio.
  • 1-2 years of experience in medical-surgical nursing, with a focus on home health care.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to work independently, with minimal supervision, and to prioritize tasks and manage time effectively.
  • Valid driver's license and CPR certification.
