Job Title: Care Coordinator

We are seeking a highly motivated and compassionate Care Coordinator to join our team at Lutheran Social Services of WI and Upper MI. As a Care Coordinator, you will play a vital role in helping families access the necessary services to support their children with developmental, physical, or severe emotional disabilities.

Job Summary:

The Care Coordinator will serve as the main point of contact for families, providing guidance and support in navigating the Children's Long-Term Support (CLTS) Waiver program. This position requires strong communication and interpersonal skills, as well as the ability to work collaboratively with families, service providers, and other stakeholders.

Key Responsibilities:
  • Assess the needs of children and families, and develop individualized service plans to support their goals and objectives.
  • Coordinate and facilitate person-centered team meetings to ensure that families receive the necessary support and resources.
  • Provide education and support to families on the CLTS Waiver program, including eligibility requirements, service options, and benefits.
  • Collaborate with service providers to ensure that families receive high-quality services and support.
  • Monitor and evaluate the effectiveness of service plans, and make recommendations for changes as needed.
Requirements:
  • Bachelor's degree in a human services-related field, such as social work, psychology, or counseling.
  • One year of experience working with children and families, preferably in a human services setting.
  • Valid driver's license and reliable transportation.
  • Ability to work independently and as part of a team.
  • Strong communication and interpersonal skills.
