LPN - Community-Based Health Worker (FT, Days)

We are seeking a skilled LPN to join our team as a Community-Based Health Worker. In this role, you will work closely with our healthcare team to deliver high-quality patient care and support services.
As a Community-Based Health Worker, you will be responsible for providing nursing and administrative support to clinicians and other healthcare team members. This will include performing clinical tasks and procedures under the direction of a clinician, maintaining patient records, and responding to messages and requests from patients and other departments.
You will also be responsible for facilitating patient care and supporting patients with complex health needs in navigating the healthcare system. This will involve working collaboratively with our healthcare team to develop and implement individualized patient plans of care.
Our ideal candidate will have a strong background in nursing and experience working in an ambulatory care setting. You will be a team player with excellent communication and interpersonal skills, and a commitment to delivering high-quality patient care.
Essential Responsibilities:
- Provide nursing and administrative support to clinicians and other healthcare team members.
- Perform clinical tasks and procedures under the direction of a clinician.
- Maintain patient records and respond to messages and requests from patients and other departments.
- Facilitate patient care and support patients with complex health needs in navigating the healthcare system.
- Work collaboratively with our healthcare team to develop and implement individualized patient plans of care.
Requirements:
- Graduate of accredited school of nursing.
- Practical Nurse License (Washington) within 6 months of hire.
- Basic Life Support required at hire from American Heart Association.
- Demonstrated clinical nursing skills.
- Demonstrated interpersonal and customer service skills.
- Demonstrated communication (written and oral) skills.
- Demonstrated literacy in using computers and keyboarding.
- Ability to work independently and efficiently under pressure.
Preferred Qualifications:
- Two (2+) years of nursing experience in assigned departments clinical area (e.g. Pediatrics, FP/IM, OB/Gyn, Surgery, Allergy, Dermatology, Urology, etc.) within an ambulatory care setting.
- IV therapy experience or recent certification.
- Familiarity in documenting in an electronic medical record system.
